Revived Body now provides Home-Kits of Vitamin shots for you to self-administer right at your comfort zone. You can either buy directly from our office (Appointment required) or order online (pickup or shipping options available).

Place Your Order and Choose Your Shipping/pickup Method

Please fill out the WAIVER form for our medical staffs to review before we can fulfill your order. If approved, there will be an invoice from Revived Body LLC that will be sent separately to you for payment.
