Place Your Order and Choose Your Shipping/pickup Method
Choose the quantity of vitamins and add to the cart. Proceed to checkout. At checkout, you will not be required to make a payment until the WAIVER is filled out and approved.

Please fill out the WAIVER form for our medical staffs to review before we can fulfill your order. If approved, there will be an invoice from Revived Body LLC that will be sent separately to you for payment.
